Severe Anemia in Children with malnutrition
​
Anaemia is common in children with severe malnutrition. Severe anaemia is defined as Hb<4 g/dl (PCV < 12%) or Hb of 4 - 6 g/dl with signs of respiratory distress/heart failure.

Management
Give 10ml/kg packed red blood cells or 10ml/kg whole fresh blood slowly over 3hours.
If child has signs of cardiac failure give packed RBCs rather than whole blood.

Give frusemide IV 1 mg/kg at the start of blood transfusion.

Stop all oral intake and IV fluids during blood transfusion.

Stop blood transfusion if a child develop fluid overload.

Do not give iron in stabilization and transition phases of Treatment.

Do not repeat blood transfusion within 4 days​
